We would like to recommend Google Forms for your survey creation and data collection. This is an easy way to post questions and then give your community members a URL where they can go to answer them! Here are some step-by-step instructions: http://docs.google.com/support/bin/answer.py?hl=en&answer=87809 for creating a form/survey.
